1. Once logged in to Powerschool, each student is listed under the PowerSchool Logo.
2. Click on "Forms" from the navigation menu.
3. Choose the tab for the Forms you need to access. To find the parent/guardian consent form click on "General Forms".

4. Fill in insurance information if you choose "Yes" to having insurance. Upload an image of your insurance card. 
5. Sign and date the form.
6. You can submit for all students by selecting "Submit for Family".

Yes, by following the steps of a modified quarantine.

• Continue to mask indoors 
• Remain symptom free
• Test two times in the next 7 days. 
• How do you get your child tested?  It’s easier than you think!
  • Option 1: Complete the Parent Consent form on PowerSchool and your child will be tested at school. 
  • Option 2: Take your child elsewhere and bring proof of a PCR (not rapid) test to school.  
  • Option 3: Don’t want your child tested? That’s okay - stay home for the 10-day quarantine period and your child’s absences will be excused.

You’re welcome to accompany your child for testing.  Most parents feel comfortable after seeing how quick and easy it is.

If you are notified that your student is being placed on Modified Quarantine, you can call your school site to arrange being present when testing. 

Once you are notified that your child is on Modified Quarantine, we can take care of the testing if you complete the Parent Consent form on PowerSchool one time. 

If you do not want your student tested, do not complete the consent form. If you completed the consent form, but would like to withdraw your consent, please contact your school site. 

Results will be emailed to the email address listed under your student's contact in PowerSchool. All results we have received will be sent daily by 8:00am. 

If your student's results are positive, you will be notified via phone call from the District Nurse and receive an email with the test results. 

The first test must be the day after you receive this notice. The second test can be sometime over the next 7 days.  The school will provide both of these opportunities.  If you received this notice after 3:00 PM, and you choose not to utilize school-based testing, you have an additional 24 hours to obtain your child’s first test and bring proof of testing to your child’s school.

We cannot accept negative rapid tests due to poor accuracy.

Some junior high students may already be vaccinated, so they would not need to quarantine unless they are symptomatic. Please provide proof of vaccination.

You will receive a phone call and an email with the test results. If you did not receive the email test results please contact the District Office at 530-224-4100.

If additional exposure occurs, you will be notified and your student will be tested again as dictated by the updated modified quarantine.

If you completed the consent form, but would like to withdraw your consent, please contact your school site. 

If you did not receive an email with your student's results, it could be one of the reasons below.

1. You do not have an email address listed in your student's contact in PowerSchool. If you do not, please contact the District Office at 530-224-4100.
